Retail Gift Aid Declaration Form

Help us to make your donation go even further so that we can treat even more sick and injured pets! Using Gift Aid means that for every pound spent on the items you donate, PDSA can claim an extra 28p from HM Revenue and Customs.

How? It couldn't be simpler - if you are a UK tax payer, just fill in your details in the form below and you will be signed up for Gift Aid. You will be allocated a Donor ID number. Make a note of this number so that you can use it while you await your Gift Aid Donor Card which we will post to you. Label any bags of donated goods with this number (using stickers provided in the PDSA leaflet where possible) so that they can be clearly identified as Gift Aid items.

We sell the goods on your behalf and because you have signed up for Gift Aid, we can claim an extra 28p on EVERY pound raised through the sale of your items.

Please provide your email address if you are
happy for us to email you in response to this online form.

  Why?

By ticking the box below I am confirming:

  • That I wish PDSA Trading Ltd to act as my agent in selling the goods I have brought into the shop
  • That I am a UK Taxpayer and I agree to PDSA claiming tax on all donations I have made for the six years prior to this year (but no earlier that 6th April 2000), and to treat all donations I make from the date of this declaration as GiftAid donations (until I notifiy you otherwise)
  • That I pay an amount of income/or capital gains tax at least equal to the tax we reclaim on your donations in the appropriate tax year (currently 28p for each £1 you give)

I agree to the above statement (please tick here)

Note: The Inland Revenue has asked us to inform you that "you must pay an amount in income tax or capital gains tax at least equal to the tax we reclaim on your donations."

In simple terms the Inland Revenue is asking you if you have paid enough tax to cover what PDSA will reclaim - almost all tax payers will have done so.
